Minnesota's Community First Services and Supports

The Community First Services and Supports Program is a new Medicaid program that is replacing the Personal Care Assistance Program. In Minnesota, this program is known as Medical Assistance. Those eligible for this program can become paid providers of services. Once the new program is in place, it will replace the Personal Care Assistance Program and Consumer Support Grant.

The new program will offer a range of services and supports to help people with disabilities live more independently. It replaces existing programs such as the Consumer Support Grant and Personal Care Assistance (PCA). The new system will provide participants with increased control over their care and the ability to purchase approved goods. The program will also allow participants to access services offered by the Minnesota Department of Human Services.

Under the new program, individuals can develop their own person-centered care plans. Through these plans, they can choose the service providers they want, based on their needs. CFSS consultation services will also offer additional support in writing plans. In addition, the new program will give consumers more options for self-direction, which will help them live independently.

The program's goal is to help people get connected to mental health services, housing, medical care, and more. The team works with people living in Times Square in order to understand their needs and connect them to resources that can help them overcome their circumstances. As a result, the program is reducing the reliance on law enforcement and other services.

The Personal Care Assistance Program (PCA) program in Minnesota will be replaced by the Community First Services and Supports Program (CFSS). PCA will continue to provide personal care assistance, but the new program is aimed at helping the elderly and disabled live independently in their own homes and communities. The CFSS program will also provide help with essential daily living necessities such as food and clothes.
